What is Advanced Acne Scar Resurfacing and How Does It Work?
Acne scars develop when inflammation damages the deeper layers of the skin during the healing process.
As the skin repairs itself, collagen can be lost or scar tissue can form, leaving areas tethered down or creating deep, narrow indentations. Because different scar types develop in different ways, there is no single treatment that works for everyone.
At MPower Clinic, we use two advanced techniques that target the underlying causes of atrophic (depressed) acne scars:
Subcision is most effective for rolling scars and tethered scars. A specialised needle is inserted beneath the skin to carefully release the fibrous bands that pull the scar down. This allows the skin to elevate naturally whilst also creating a controlled healing response that stimulates new collagen production, gradually improving the scar's appearance over the following months.
TCA CROSS (Chemical Reconstruction of Skin Scars) is particularly effective for ice-pick scars and narrow boxcar scars. A tiny amount of high-strength TCA (trichloroacetic acid) is applied directly into each individual scar rather than across the whole face. This creates a controlled healing response within the scar itself, encouraging new collagen formation and gradually making the scar shallower over time.
Because most patients have a combination of rolling, boxcar and ice-pick scars, treatment is often tailored using both techniques during the same appointment. Depending on your skin and treatment goals, we may also recommend combining your treatment with chemical resurfacing, regenerative injectables or other collagen-stimulating procedures to optimise your final results.
Am I Suitable for This Treatment?
This treatment is most suitable for patients with depressed acne scars once their acne is well controlled.
It can effectively treat:
Rolling scars
Boxcar scars
Ice-pick scars
Mixed atrophic acne scarring
It is generally not suitable for raised (hypertrophic or keloid) scars or patients with active inflammatory acne.
During your consultation, we'll assess your skin and discuss whether additional treatments—such as chemical resurfacing, microneedling, regenerative injectables or skin boosters—may further optimise your results.
How Many Treatments Will I Need?
Every patient's acne scarring is unique, so there is no single treatment plan that suits everyone.
Some patients notice a meaningful improvement after just one session, whilst others benefit from 2–4 treatments depending on the severity and type of scarring. Treatments are typically performed every 6–12 weeks, allowing sufficient time for collagen remodelling before the next session.
Because collagen continues to form for several months after treatment, results develop gradually rather than immediately. Your treatment plan will be reviewed throughout your course to ensure the best possible outcome.

SIDE EFFECTS
Redness, swelling, bruising, temporary scabbing, tenderness
Possible Side Effects
Normal: Mild redness, swelling, tenderness and bruising are common following treatment, particularly after subcision. If TCA CROSS is performed, small white frosting immediately after treatment followed by tiny scabs over the treated scars is expected. These naturally heal and separate over the following days as collagen remodelling begins.
Rare: Infection, prolonged redness, delayed healing, temporary changes in skin pigmentation, prolonged bruising, scar widening. These risks will be discussed during your consultation.
Urgent: Contact the clinic immediately if you experience severe or worsening pain, rapidly spreading redness, increasing swelling, pus or discharge, fever, extensive blistering, or any other signs of infection or delayed healing.
Pre-Treatment Advice
• Avoid retinoids, exfoliating acids, active skincare products, tanning and excessive sun exposure for 5–7 days before treatment.
• Inform us of any active acne, skin infections, recent aesthetic treatments or history of cold sores.
• Attend with clean, product-free skin.
• If you feel unwell on the day of treatment, please contact your practitioner to reschedule.

Post-Treatment Advice
• Redness, swelling, tenderness, bruising and small scabs (if TCA CROSS is performed) are normal and will gradually improve as your skin heals.
• Avoid picking scabs, active skincare products, excessive heat, strenuous exercise and direct sun exposure while your skin recovers.
• Keep the treated area moisturised and wear SPF 50+ daily to protect the healing skin.
